About the Role This is an exciting opportunity to join our well-established and proudly northern Case Management company. We are determinedly client-focussed, experienced, and work hard to create a friendly team approach. Our business is growing, and we are looking for new professionals to expand our activities and join our supportive team. You will have the support of our highly experienced team, including our very efficient and reliable administration service. The role is home-based, requiring from 28 to 37 hours per week, during which time you will manage your client caseload with visits to your clients' homes within your geographical area. Home based working does not mean you are working alone, as you will work closely with, and be supported by, managers/supervisors, colleagues and our administration service. We meet regularly, virtually, and often face-to-face, with the aim of prioritising the well-being of both clients and staff without compromise. There are opportunities to spend time working side by side with colleagues who are based predominantly in the Newcastle and Sheffield areas. We provide supervision and training, and are proud of our identity and reputation in the care community, as highly skilled, effective, and very much client-centred. We have an excellent mentoring & development programme for professionals new to case management. About you You should be based in Northern England (Tyne and Wear, Cumbria, Northumberland, Durham, Teesside, Yorkshire, Derbyshire, Nottinghamshire). We are looking for someone who is capable, experienced, flexible, and enthusiastic. More specifically we are seeking someone with the following skills and abilities. For those new to case management, you should have at least three years professional experience. For those already in case management, you should have a professional qualification social work, health or social care, or as a rehabilitation professional, and have at least two years case management experience For experienced case managers, you should have at least five years case management experience, and ideally be an Advanced Member of BABICM. For Associates, you should have at least five years case management experience and an ability to work autonomously. You should also: Have the ability to establish and maintain a positive and supportive working relationship with clients and their families. Have the proven ability to manage support packages Be able to co-ordinate multi-disciplinary rehabilitation programmes Have excellent communication skills Have very good report and assessment writing skills Hold a current DBS and are registered to the update service Have a full UK driving licence and use of a vehicle Be competent using a laptop and in the use of client recording software (training will be provided) To ensure client confidentiality in all matters relating to our clients What to do next?
